For first visits, patients without a prior diagnosis or who have never used medication should complete a Comprehensive Diagnostic Evaluation. For patients who have a prior diagnosis and have used medication or are actively taking medication, starting with a New Patient Visit is recommended.
After initial visits are completed, most patients are seen once per month for a 30-minute follow-up.
Some appointments require more time when discussing test results, engaging in therapy, meeting with family members, etc. These will be determined with your provider and will be specific to your treatment goals.

All cancellations require at least 72 hours' notice to avoid a cancellation fee. Any cancellations made within 72 hours of your appointment will incur the late cancellation fee (the full cost of the session).
Notification must be made to your provider and the administrative assistant via the CharmHealth Patient Portal messaging center and/or by calling 585-204-0066.

Our practice does not work directly with insurance and is considered out-of-network. Appointments with your provider are paid out of pocket. Your paid invoice for our services can serve as a Superbill that you can submit directly to your insurance.
Laboratory services and prescriptions will be covered in accordance with your specific insurance coverage. Please inquire directly with your insurance company to determine laboratory and prescription costs.
What is a Superbill?
A superbill is a detailed receipt used in healthcare that patients can submit to their insurance company for reimbursement after receiving out-of-network services.
It typically includes:
Provider information (name, credentials, NPI, address)
Patient information
Dates of service
CPT codes (what services were performed)
ICD-10 codes (diagnoses)
Fees charged and paid
In private or out-of-network practices, instead of billing insurance directly, you give the patient a superbill. The patient then files it with their insurer to seek reimbursement under their out-of-network benefits.
How to submit and use superbills?
Patients can submit by:
Uploading it through their insurance portal/app (most common now)
Emailing it to their insurer
Faxing or mailing it to the contact information on the back of the insurance card
Using reimbursement apps (e.g., Reimbursify, Thrizer)
